Clinical and diagnostic significance of il-1b and il-33 and their relationship with biomarkers of myocardial stress nt-probnp and sst2 in children with congenital heart defects

Abstract

Background: Congenital heart defects (CHDs) are associated with systemic inflammation and myocardial stress, highlighting the need for informative biomarkers.


Objective: To determine the diagnostic value of IL-1β and IL-33 and their relationship with NT-proBNP and sST2 in children with CHDs.


Materials and Methods: From 2024 to 2026, 232 children were examined: 202 patients with CHDs, including 92 with cyanotic and 110 with acyanotic defects, and 30 apparently healthy controls. IL-1β and IL-33 were measured by ELISA, NT-proBNP by immunochemiluminescence assay, and sST2 by ELISA.


Results: IL-1β levels were 2.9±0.4 pg/mL in controls, 23.7±4.8 pg/mL in cyanotic CHDs, and 16.3±2.6 pg/mL in acyanotic CHDs. IL-33 levels were 2.3±0.3, 19.2±3.6, and 14.5±2.2 pg/mL, respectively. Positive associations were found between the cytokines and NT-proBNP/sST2; modified ultrafiltration was accompanied by a greater postoperative reduction in the studied markers.


Conclusion: IL-1β and IL-33, combined with NT-proBNP and sST2, may support integrated assessment of inflammatory activation and myocardial stress in children with CHDs.
