Maqsad. 10 kg dan kam vaznga ega chaqaloqlarda qorinchalararo to‘siq nuqsonini (VSD) transkateter usulida yopish kichik bemor o‘lchamlari, qon tomir kirish yo‘llarining cheklanganligi va asoratlar xavfining yuqoriligi sababli texnik jihatdan murakkab hisoblanadi. Faqat arterial kirish yo‘li usuli jarayonni soddalashtirish orqali afzalliklar berishi mumkin. Ushbu tadqiqot 10 kg dan kam vaznga ega chaqaloqlarda faqat arterial kirish yo‘li orqali VSD ni transkateter yopishning xavfsizligi, samaradorligi va o‘rta muddatli natijalarini baholaydi.
Materiallar va usullar. O‘rtacha yoshi 14,5 oy (IQR 10–21,5) va o‘rtacha vazni 8,95 kg (IQR 8–9) bo‘lgan 20 chaqaloqqa (9 o‘g‘il bola [45%], 11 qiz bola [55%]) faqat arterial kirish yo‘li orqali VSD transkateter yopilishi amalga oshirildi. O‘rtacha nuqson o‘lchami 4,5 mm (IQR 3,5–4,95), qurilma diametri esa 7×5 dan 8×6 mm gacha (o‘rtacha 7×5) bo‘ldi. O‘rtacha ftoroskopiya dozasi 19 mGr (IQR 13–29,5), jarrohlik jarayoni o‘rtacha 50 daqiqa (IQR 40–60) davom etdi. Kuzatuv muddati o‘rtacha 15 oy (IQR 12–22) bo‘ldi.
Natijalar. Jarayon 100% muvaffaqiyatli yakunlandi. 1 yil ichida 15% bemorda qoldiq shunt kuzatildi. 30% bemorda trikuspital regurgitatsiya qayd etildi, aortal regurgitatsiya holati aniqlanmadi. Jiddiy asoratlar yoki o‘lim holatlari qayd etilmadi.
Xulosa. 10 kg dan kam vaznga ega chaqaloqlarda faqat arterial kirish yo‘li orqali VSD transkateter yopilishi amalga oshirilishi mumkin va xavfsiz bo‘lib, o‘rta muddatli ijobiy natijalarni beradi. Ushbu usul jarayon vaqtini va kontrast moddaga tushadigan yukni kamaytirishi mumkin, bu esa uni kichik chaqaloqlar uchun jozibador variantga aylantiradi.
