The Influence of Anesthetic Management on the Condition of Newborns Delivered by Cesarean Section from Mothers with Severe Mitral Stenosis

Abstract

The results of the examination of 38 premature newborns (19 patients in each group) are presented, with the aim of a comparative assessment of the condition of newborns delivered by cesarean section (CS) from mothers with severe mitral stenosis (MS) under the use of a traditional variant of general multicomponent anesthesia (GMA) with mechanical ventilation (MV) and general combined anesthesia (GCA) based on epidural blockade (EB) with reduced concentrations of local anesthetics.


It was established that GCA with MV on the background of EB has a less pronounced negative effect on newborns compared to GMA with MV, allowing for better preservation of the child’s adaptive capabilities during the early period of adaptation to extrauterine conditions, despite the extremely unfavorable initial background (prematurity; significant reduction of maternal coronary reserves due to severe MS (1.9–1.1 cm²)).
