Quality of life and functional outcomes after surgical correction of pubic diastasis in children with bladder exstrophy

Abstract

Objective: To assess the functional outcomes and quality of life of children after surgical correction of pubic diastasis in bladder exstrophy.


Materials and Methods: The outcomes of 44 children operated on in 2021-2026 were analysed. Quality of life and satisfaction were assessed by a questionnaire completed by the parents of 41 children, and in 11 older children additionally by the child's own rating; urinary continence, bladder capacity, infection rate and impact on daily life were taken into account.


Results: Fully or mostly satisfied with the operation were 39 of 41 parents (95.2%). Bladder capacity increased from zero baseline values to a median of 38.5 mL on ultrasound and 47 mL on cystography (p < 0.005), continence reached a median of 40 minutes, and urinary tract infections were absent in 84.8% of children. The children's own rating was stricter than the parental one: maximum satisfaction was reported by 36.4% of children versus 75.6% of parents.


Conclusions: Correction of the diastasis improves functional status and the family's perception of the result, yet the children's stricter self-assessment and residual incontinence emphasise the need for long-term follow-up.
